Many lateral ankle sprains produce instability of the ankle joint which can last a lifetime – two big issues arise. One is that the medial ligaments and soft tissue are in a shortened protective position and must be addressed with gentle cross fibre friction, soft tissue mobilising and eversion of the calcaneal bone. Secondarily the navicular head (bone in the ankle) can be slightly displaced anteriorly and needs to be rocked back into position via way of standing dorsi-flexion movement. This can be achieved by thumb over thumb and compression into the ankle from an anterior/superior to posterior/inferior glide whilst your client bends and flexes at the knee. Don’t forget to mobilise the mid-foot to release any additional restrictions on the 1st cuneiform bone which is often the tightest point in the anatomical stirrup.
